DVA Community Nursing
At abbaCare, we proudly offer a range of in-home and community support programs through the Department of Veterans' Affairs (DVA) to help eligible veterans and war widows/widowers (Entitled Persons) maintain independence and continue living comfortably at home.
Our Community Nursing services are tailored to meet the clinical and personal care needs of entitled persons, including veterans and war widows who hold a Gold Card. Veterans with a White Card may also be eligible if the condition requiring nursing is recognised by DVA as war-related. The goal of this program is to enhance independence, improve quality of life, and ensure positive health outcomes for veterans and their widows by preventing early hospital or residential care admission through access to high-quality nursing services at home.
How to Access DVA Community Nursing
At abbaCare, we guide you through every step of the process. To access DVA community nursing, you will need a referral from your GP or Hospital Discharge Planner. Once the referral is made, our DVA Care Coordinator will work with you to schedule a convenient time for a comprehensive nursing assessment, conducted by one of our experienced and dedicated Registered Nurses.
We offer a broad range of community nursing services, all designed to promote the health and well-being of entitled veterans and widows. The services provided are based on each individual’s assessed care needs, ensuring a personalised approach to support.
