Your NDIA planner will contact you to schedule a suitable meeting time. During this meeting you will discuss your goals, work through the support and services required and begin to develop your individualised plan.
Your plan will be based on your goals, circumstances, needs and objectives and can include things like your living arrangements, employment, transport, social activities and aspirations.
You and your family are fully involved in discussing what works best for you in putting your plan into action.
You will review your plan with your NDIA planner and abbaCare at agreed review dates, approximately every 12 and 24 months, to see what’s working and what’s not. If your circumstances change, you can request a review at any time. The end of plan review is based on the length of the NDIS plan.
